Payment will be required according to the clinic’s fee schedule at the time of your visit. A receipt may be provided or sent to you so that you can verify your eligibility for reimbursement with your insurance company. Please note that you are responsible for confirming coverage and submitting any reimbursement claim to your insurer.

Please note that a one-time $75 patient file opening fee applies to the first appointment only.

Cancellation Policy: To allow another patient to benefit from this appointment time, any cancellation made less than 24 hours in advance, as well as any missed appointment, will result in a $75 fee.
