You will need to create your profile and complete the required medical forms before your consultation.

Preparation: Please have your medical documents available (medication list, recent test results, and medical history) to complete your online file.

Important conditions: To ensure safe and effective care, the consultation may be limited, or your appointment may be postponed if your online file and required medical information are not completed before your visit.
